Essential Functions

  • Facilitate 748 hours per year (17 hours a week x 44 weeks, at least 8.5 hours per week must be directly in Cumberland AAI) of 1:1 instruction with their assigned caseload working on youth established goals in the Transition/Independent Living plan at the level of service determined by the Service level needs assessment. 
  • Ensure the timely & accurate completion of all required paperwork (includes documentation requirements) per the AAI Paperwork Guidelines/Manual document and the approved program description.
  • Ensure timely updates to all members of the client's team per the guidelines listed in the approved program description.
  • Represent Valley Youth House in a professional manner at all Court hearings, professional meetings and public events.
Minimum Qualifications

  • A Bachelor's degree in Social Work or related field. If the degree is in an unrelated field then the candidate must have at least 1 year of experience in the social services field.
  • One or more years experience working with youth and young adults or providing case management services.
  • A valid driver's license, a motor vehicle report that meets agency requirements and reliable transportation that can safely transport clients if needed.
  • Ability to work a hybrid schedule that includes evenings.
  • Ability to complete the pre-employment physical and drug test requirements.
Physical Requirements:

  • Sedentary work that primarily involves sitting/standing. X Occasionally 
  • Light work that includes moving objects up to 20 pounds. X Occasionally 
  • Medium to heavy work requiring walking X Constantly
  • Light work that includes stairs and narrow access points X Occasionally 
  • Light work that requires long distance driving X Occasionally
